Equal-HousingBehavioral Health Initiatives, Inc. (BHI) Residential Living Facilities located throughout West Tennessee provide affordable, permanent housing to individuals who qualify by meeting the guidelines set forth in the contract with TDMHDD and in accordance with HUD standards. BHI is a Fair Housing entity.

BHI has three levels of housing: Supportive Living, Congregate Independent Living, and Independent Apartments.

 

Independent Apartments:

Independent Apartments are for individuals who can live completely independently. They must be able to take medicines, make and remember appointments, arrange transportation, cook, clean, and do everything completely on their own. These apartments offer affordable housing to individuals who are low-income and diagnosed with a mental illness.

The Phoenix Project located in Memphis, TN is dedicated to chronically homeless individuals and is a member of Memphis’ Coordinated Entry Process.

 

Congregate Independent Living:

BHI's Independent Living homes are not staffed, but residents do share the home with up to four other mental health consumers with all household duties being shared equally. Each resident’s case manager provides support services as needed. Congregate Independent Living residents also need to be able to cook, clean, make appointments, arrange for transportation, and take medications on their own. Independent homes are low rent and include basic utilities.

Supportive Living:

BHI has group homes which are staffed 24 hours a day, 7 days a week. A staffing pattern of 1 staff to every 8 residents enables the highest level of self-care possible for each resident. Staff duties include training, assisting, monitoring, and interacting with each resident. Staff cooperates with mental health case managers in order to assist with physical and mental health providers. BHI promotes its homes as having a family-like atmosphere and encourages any and all suggestions from our residents on ways to improve our services.

Admission Criteria

  • Applicant must be diagnosed as mentally ill.
  • Applicant must be at least 18 years of age.
  • Applicant must be mental health and med compliant.
  • Applicant must be able to perform basic self-help activities.
  • Applicant must be able to care for their own possessions and maintain bedroom and other living areas in a reasonable state of orderliness and cleanliness.
  • Applicant must be able to recognize dangers or threats to personal safety.
  • Applicant must be able to live comfortably within any limitations in the structure of the facility
  • Applicant must be able to maintain appropriate behaviors tolerable to the community.
  • Applicant should be able to provide for his/her own medical expenses and personal items.
  • Applicant must not be a danger to oneself or others.
  • Applicants must have a physical examination within 30 days of admission to the program to ensure that any medical issues found may be addressed in a timely fashion.
  • Applicants are expected to have adequate finances to cover the cost of move-in, including one month’s pro-rated rent, security deposit, personal items, and spending money. Security deposits may be paid in monthly installments, with the amount being determined prior to move-in.
  • Applicant must be able to care for any medical condition that requires daily care (i.e. insulin injections, catheter, colostomy, etc.)

 

Referral Process
Individuals who are appropriate for the Housing Program can secure a referral form by clicking the download link below or in person from the corporate office of Behavioral Health Initiatives, Inc. Submission of a referral form places the person on the appropriate waiting list. An assessment for housing will be completed as a follow-up to the referral when that person is next on the list.

During this assessment, applicants will be required to sign an authorization for BHI to conduct a background check. This background check is completed in an effort to secure the safety of residents, staff, and the general community. Each applicant’s mental health provider will be contacted to verify their eligibility and cooperation with outpatient services. Once this has been completed, the Housing management team will meet to discuss appropriate placement.
